The Centers provides services from six locations in Cuyahoga County, offering daytime, evening, and same day appointments. Our staff also frequently provides services in community locations and client homes. Our clients are active participants in their treatment and care coordination plans.
The goal of our health and wellness program is to engage clients in the recovery process and help them manage their mental and physical health, while also managing the stressors of daily living. Services are focused on the individual’s ability to succeed in the community and to identify and access needed services. Clients and staff work together to achieve positive outcomes and stability in housing, health symptoms, hospitalization, connections to others, and meaningful activity/employment.
